The current situation and influencing factors of patient perception for humanistic care in 30 provincial hospitals

Objective To understand the current status and influencing factors of patient perception for humanistic care in China hospitals,and to provide a basis for developing nursing humanistic care measures and improving the quality of nursing humanistic care services. Methods A total of 30,099 outpatients and inpatients from 107 hospitals in 30 provinces(autonomous regions and municipalities) from July to August 2022 as survey subjects. A general information questionnaire and the Relational Caring Questionnaire-Patient Form were used for a cross-sectional survey,and a single-factor analysis was used to analyze the influencing factors of patient relationship care. Results Finally,29 108 valid questionnaires were collected,and the effective questionnaire recovery rate was 96.7%. The patient evaluation of relationship care was(65.72±8.61) points. Single-factor analysis showed that gender,age,marital status,children’s situation,education level,occupation,place of residence,average family income,medical insurance type,visiting department,and location of the visiting hospital,and whether or not surgery were influencing factors of patient relationship care(P<0.05). Conclusion The evaluation score of caregiver-patient relationship care among Chinese hospital patients is above average,but there is still room for improvement in western and rural regions,seriously ill and outpatient patients,low-income and low-medical insurance reimbursement populations,and non-surgical patients. Medical institutions at all levels should optimize and improve nursing humanistic care services based on influencing factors,and further enhance patients’ perception of nursing humanistic care.

Objective To develop the Holistic Human Caring Perception Scale for clinical nurses and to test its reliability and validity. Methods On the basis of documents retrieval and interviews,the pre-test scale was constructed through Delphi method. In April 2022,20 nurses in Wuhan were selected for a pre-survey to test the readability of the scale. From April to May 2022,a total of 501 clinical nurses in hospitals in Xianning and Tai’an were selected as the investigation subjects. Item analysis and reliability and validity test of the scale were conducted. Results The formal holistic human caring perception scale for clinical nurses with 6 dimensions and 40 items was formed. 6 common factors were extracted by exploratory factor analysis and the cumulative variance contribution rate was 79.05%;confirmatory factor analysis showed that the fitting index of the scale structure model was good. The S-CVI of the scale was 0.94 and the I-CVI was 0.78~1.00;the Cronbach’s α coefficient of the scale was 0.98 and the test-retest reliability was 0.87. Conclusion The reliability and validity of the Holistic Humanistic Caring Perception Scale for clinical nurses are good,which can comprehensively reflect the caring perception of clinical nurses.

Sinicization of the Tolerance for Mental Pain Scale and its reliability and validity in young adults living with HIV/AIDS

Objective To translate and cross-culturally adapt the English Tolerance for Mental Pain Scale (TMPS-10) into Chinese and test its reliability and validity in young HIV/AIDS patients. Methods We sinicized the TMPS-10 following the forward translation,back translation,cultural adaptation,and pre-survey according to the Functional Assessment of Chronic Illness Therapy translation methodology(FACIT). Evaluation of content effective-ness was conducted through 2 rounds of expert consultations. From May to August 2022,334 young HIV/AIDS patients who were admitted to an HIV specialty clinic of a tertiary A hospital in Changsha were selected to test the reliability and validity of the scale. Results The Chinese version of TMPS-10 includes 2 dimensions with a total of 10 items. The content validity index of the total scale is 0.975,and the content validity index of each item is between 0.875-1.000. Exploratory factor analysis resulted in a 2-factor structure,explaining 65.719% of the total variance. Confirmatory factor analysis showed a good model fit. The Cronbach’s α for the total TMPS-10 score was 0.832,and 0.894 for the managing the pain dimension,and 0.856 for the enduring the pain dimension. Conclusion The Chinese version of TMPS-10 has good reliability and validity,which can be used to evaluate the tolerance for mental pain of young HIV/AIDS patients in China.

Posttraumatic growth in breast cancer patients and their spouses and its relationship with positive psychological capital

Objective To investigate the post-traumatic growth status of breast cancer patients and their spouses,and to explore the effect of the positive psychological capital of patients and spouses on their own and each other’s post-traumatic growth. Methods From March to June 2021,a total of 130 pairs of female breast cancer patients and their spouses in a tertiary hospital in Tianjin were investigated by convenience sampling method and completed the investigation,using the general information questionnaire,Posttraumatic Growth Inventory,and Positive Psychological Capital Scale. Results The posttraumatic growth scores of breast cancer patients and their spouses of(2.43±1.11)(2.34±1.27) were significantly lower than the theoretical median value of 3.5,which were in the lower middle level(P<0.001). The total score of positive psychological capital of patients was lower than that of spouses(P=0.033). In the dimension of patients’ positive psychological capital,the level of resilience was relatively low. Subject-object interdependence model analysis showed that the positive psychological capital of patients and their spouses positively predicted their posttraumatic growth(Ps<0.001). Positive psychological capital positively predicted the posttraumatic growth of spouses in breast cancer patients(P<0.05),but the partner effect on spouses was not significant(P>0.05). Conclusion The posttraumatic growth level of breast cancer patients and their spouses is in the lower middle level. The level of patients’ resilience is relatively low. The partner effect of positive psychological capital of patients and spouses on posttraumatic growth is asymmetric,and the support of positive psychological capital of spouses and patients’ posttraumatic growth is insufficient.

Objective To learn about current status of knowledge,attitude and practice of humanistic care in Chinese nurses and their influencing factors,so as to provide references to the mangers to accordingly improve their knowledge,attitude and practice. Methods From July to August,2022,a general information questionnaire and a clinical nurse questionnaire of knowledge,attitude and practice were sent out and filled by 15 727 nurses from 107 hospitals in 27 provincial administrative regions of China. Results 15,653 nurses completed the investigation. The total score of knowledge-attitude-practice was 84.19±12.98. The scores of knowledge,belief or attitude,and standard score of practice were 80.22±15.61,83.55±15.17,and 87.92±12.70,respectively. The influencing factors were gender,title,department,satisfaction degree with work,emphasis degree of self-care,received training of humanistic care or not,supporting degree from family to their work,relationship with colleagues,satisfaction degree with salary,and having worked or not in a trial ward of humanistic care(P<0.001). Conclusion The overall humanistic care of Chinese nurses currently stands at medium level. The knowledge of humanistic care is relatively inadequate. The managers should strength nurse humanistic care training,give organizational support,build a better department atmosphere,and improve nurses’ satisfactions to their work,salaries,and relationship with colleagues,so as to improve nurses’ humanistic care of knowledge,attitude and practice.

Discussion on the starting time of shoulder and neck rehabilitation exercise in patients after endoscopic thyroid cancer surgery through gasless transaxillary endoscopic thyroidectomy

Objective To investigate the optimal starting time,safety and effectiveness of shoulder and neck functional rehabilitation exercise for patients undergoing gassless transaxillary endoscopic thyroidectomy(GTET). Methods 175 patients who underwent GTET were selected by convenient sampling,with 121 patients in 2 test groups(61 patients in the test group 1 and 60 patients in the test group 2) and 54 patients in a control group. The starting time of postoperative shoulder and neck rehabilitation training in the control group,test group 1 and test group 2 were postoperative 7 days,postoperative 48 hours and 2 hours after general anesthesia wakening,respectively. The analysis among 3 groups included the amount of drainage(ml),the retention time of drainage tube (h),the postoperative hospital stay(d),the cases of bleeding,the pain score of incision(neck and armpit) at postoperative day 1 and day 3,the range of shoulder and neck movement and the quality of life. Results There was no statistically significant difference in term of drainage amount,retention time of drainage tube,bleeding cases,hospital stay and pain in neck and incisions at postoperative day 1 and day 3 among the 3 groups(P>0.05). The motion range of cervical extension,lateral cervical flexion,cervical rotation,shoulder abduction,shoulder adduction,shoulder flexion and shoulder extension among different groups were statistically significant 1 month after surgery(P<0.001). In term of quality of life(QOL),there were statistically significant differences in aspects of stiffness,pain,numbness of the neck or shoulder,and the impact on leisure and entertainment(P<0.05). Conclusion Starting shoulder neck rehabilitation exercise 2 hours after awake under anesthesia in patients undergoing GTET would not cause bleeding and affect the comfort,and could safely and effectively improve the early QOL of patients in the early postoperative period.

The nursing care of a patient with severe toxic epidermal necrolysis complicated by acute respiratory distress syndrome was summarized. Key points of care:personalized sedation and analgesia,management of prone ventilation,extracorporeal membrane oxygenation combined with continuous renal replacement therapy,protection of damaged mucosa,refined skin management,prevention and control of infection,etc. The patient was transferred out of the department after 10 days,and after 5 months of regular follow-up,the patient recovered well.

Objective To explore the different application types of combined exercises in patients with type 2 diabetes,and to clarify the characteristics of different combined exercises. Methods Guided by the scoping review framework proposed by Arksey et al.,a literature search was performed in the Chinese and English literature Databases. The retrieval period was from the establishment of the database to February 26,2022,and relevant information such as research subjects,sample size and monitoring indicators of the included literature was extracted. Results A total of 18 articles were included. Combined exercises can be divided into different combined types by exercise types(same,different),intensity(high intensity intermittent,moderate-intensity continuous,mixed intensity and different intensities of resistance) and sequence(dual-task,interactive and sequential). When applied to patients with type 2 diabetes mellitus,combined exercises not only present a single aspect of characteristics,but also intersect with each other. Conclusion The 3 combined exercises have a significant impact in terms of increasing exercise variety,adjusting exercise rhythms and enhancing exercise effects. However,there are still limitations in exercise selection and effect evaluation. Therefore,there is still a need to further explore how to develop normative operational standards,intelligently recommend personalised programmes and identify suitable populations in combined exercises.

To summarize the nursing experience of a patient with successful pregnancy and delivery after heart-lung transplantation. Nursing points include:establishing a multidisciplinary team of perinatal maternal-fetal cardiovascular disease to escort patients for successful delivery; individualized pre-pregnancy counseling to choose the best timing for pregnancy; adjustment of the immunosuppressive regimen during pregnancy and close monitoring of blood drug concentrations; refined pregnancy management,prevention of serious complications; rational selection of the mode of delivery and strict monitoring during labor;postpartum care and feeding guidance; psychological counseling to enhance patient confidence. After careful nursing and guidance,the patient successfully conceived and gave birth to a healthy baby. The mother and baby were discharged safely 5 days after the operation,and they were in good condition after 3 months of follow-up.

Analysis of influencing factors and impact path of compassion fatigue in nurses from maternity hospitals in Jiangsu

Objective To investigate the influencing factors of compassion fatigue(CF) among nurses in maternity hospitals,and then to further explore the impact path of factors. Methods 715 nurses in maternity hospitals from Jiangsu were investigated by the cross-sectional survey. A self-developed questionnaire,the Five-Factor Inventory,the Social Support Rate Scale,the Psychological Capital Questionnaire-Revision,and the Compassion Fatigue Scale were adopted to collect data. Results The number of children,personality of neuroticism and agreeableness,social support,and psychological capital are the influencing factors of CF(P<0.05),while professional titles,length of services,labor relations,personality of extraversion and openness,social support and psychological capital influence compassion satisfaction(CS)(P<0.05). In structural equation model,the indirect effects of neuroticism on CF or CS existed(ES=0.062,-0.420). Social support and psychological capital play a partially mediating role in neuroticism and CF(ES=0.017),accounting for 27.42% of the total indirect effects. They also play a completely mediating role in neuroticism and CS(ES=-0.112),accounting for 26.67%. Positive personality had a significant indirect effect on CF or CS(ES=-0.158,0.623),and social support and psychological capital serve as a completely mediating role between them (ES=-0.028,0.102),accounting for 17.72% or 16.37% of the total indirect effects. Conclusion Nursing managers should take effective measures tailored to personality traits to help nurses meet high levels of social support and psychological capital,so as to promote CS and reduce CF.

The effect of 9 types of exercise in improving the dialysis adequacy on patients undergoing hemodialysis:a network meta-analysis

Objective To systematically evaluate the effect of different types of exercise in improving the dialysis adequacy of patients undergoing hemodialysis. Methods The databases of PubMed,Cochrane Library,Embase,Web of Science,WanFang,CNKI,VIP,and CBM were searched for the articles related to the effectiveness of exercise on the dialysis adequacy of patients undergoing hemodialysis,from inception to 1st January,2022. After literature screening and data extraction,Cochrane risk of bias tool 2.0 was used for quality evaluation. RevMan 5.3 and STATA 16.0 software were used for statistical analysis. Results 30 studies with 1 216 participants and 9 types of exercise were included. The results of the network meta-analysis showed that intradialytic combined aerobic and resistance exercise and intradialytic aerobic exercise were the top 2 types in improving the single pool clearance index on hemodialysis patients. Intradialytic aerobic exercise and intradialytic combined aerobic and resistance exercise were the top 2 types in improving the urea reduction ratio of hemodialysis patients. Conclusion The dialysis adequacy of hemodialysis patients can be improved by a variety of exercise,especially intradialytic combined aerobic and resistance exercise,and intradialytic aerobic exercise.

Concept analysis of parental perceptions of child vulnerability

Objective To clarify the concept of parental perceptions of child vulnerability. Methods We searched studies on parental perceptions of child vulnerability from websites and databases,including CNKI,Wanfang,VIP,CBM,PubMed,CINAHL,Embase and Web of Science. We selected relevant papers from the inception of databases to March 2022 according to inclusion criteria. Rodgers evolutionary method of concept analysis was used. Results A total of 33 articles were retrieved. 4 attributes were identified on parental perceptions of child vulnerability,including interacting between the child and their parents,having experienced or being currently in a traumatic event,the child in a healthy or stable state at present,negative psychological reaction and behaviors of parents and the child. The prerequisites and factors include the child and their parents,hospital factors and social factors. Parental perceptions of child vulnerability easily affect the healthy growth of the child,reduce the ability of parents to take care of the child and waste medical resources. The measurement indicators are mostly the level of parental perceptions of child vulnerability,the level of parental protection and so on. Conclusion The concept attributes of parental perceptions of child vulnerability were identified by concept analysis method. Researchers can carry out more comprehensive and in-depth research,combining the attributes of parental perceptions of child vulnerability.

To summarize the nursing experience of a patient with acute ST-segment elevation myocardial infarction complicated with epilepsy. The main nursing care are as follows:initiating a multidisciplinary response team to formulate the best diagnosis,treatment and nursing plan;identifying epilepsy seizure and emergency nursing care;strengthening disease observation and nursing to prevent north-south syndrome;conducting individualized and progressive rehabilitation training to prevent ICU acquired weakness. After meticulous treatment and nursing care,the patient experienced the improvement and was discharged.

Best evidence summary for management of leg cramps during pregnancy in pregnant women

Objective To search and obtain relevant evidence of leg cramps management in pregnant women,and to provide the evidence for clinical practice. Methods We systematically searched relevant guidelines websites,professional associations websites,as well as Cochrane Library,PubMed,Embase,CNKI,CBM and other databases for all evidence on management of leg cramps during pregnancy,including guidelines,clinical decision-making,evidence summaries,system reviews,expert consensuses,randomized controlled trials from the inception of databases to December 2021. The literature was evaluated and rated for quality by 2 researchers independently. Results Totally,13 articles were enrolled,including 1 guideline,4 clinical decisions,5 systematic reviews and 3 randomized controlled trials. A total of 24 pieces of evidence were summarized,including evaluation and diagnosis,risk factors,basic treatment principles,acute treatment,non-drug intervention and drug intervention. Conclusion This study summarized and formed the management plan of leg cramps in pregnant women,including its assessment and diagnostic points,risk factor management,interventions,etc. Evidence translation should be combined with clinical context and pregnant women’s willingness to select the best evidence to reduce the incidence of leg cramps in pregnant women and improve the quality of life during pregnancy.

A multicenter survey on the status and demand of humanistic nursing training for nurses

Objective To understand the status and training needs of humanistic nursing training for nurses,so as to provide bases for the development of humanistic nursing training programs and targeted humanistic nursing in-service training for nurses. Methods From August to September 2021,2 3707 nurses from 28 provinces were investigated by convenient sampling. A self-designed questionnaire was used and launched through the platform of Wenjuanxing. Results 99.71% of nurses had participated in at least a humanistic nursing training;54.61% of nurses had 26 training experiences;the training rate and demand rate of 26 humanistic nursing training contents were 74.63%~98.30% and 88.75%~96.35%,respectively. The main ways for nurses to participate in training were pre-job training(77.08%),stratified core competence training(73.56%),and hospital public training(72.92%). The training forms mainly included theoretical teaching(85.96%),video learning(72.61%),and case teaching(64.17%). The majority of trained teachers were nursing managers (89.19%). Single training form (49.42%),lack of targeted content(46.78%),unsystematic training courses(37.47%),weak training teachers(36.87%) are the main problems existing in the current training. 95.02% of the nurses thought that it was necessary to establish systematic and standardized humanistic nursing practice ability training courses for clinical nurses;87.93% of the nurses thought that it was necessary to carry out humanistic nursing specialized nurse training;46.19% of the nurses chose the training mode of theory and practice. 93.54% of nurses hope to receive theoretical knowledge training online. Conclusion Humanistic nursing in-service training of nurses in China has a wide range of training content,high training rate,and great training demand. The training courses,training forms,and training teachers should be further advanced. It is urgent to standardize the humanistic nursing in-service training program and construct a humanistic nursing training mode to improve the training effect.

Objective To explore the post-traumatic growth(PTG) level and change of patients with bone neoplasms within 30 day after surgery,and the correlation with post-traumatic stress(PTS) after surgery. Methods A prospective cohort study was conducted by convenience sampling to select patients who met the criteria for inclusion and exclusion in the orthopedic ward of a tertiary hospital in Sichuan Province. The cohort was divided based on the scores of the revised version of Chinese translation of the Impact of Event Scale-Revised for patients 2 days after surgery. Patients whose scores were≥26 points would be included in the larger PTS group,and <26 points would be included in the smaller PTS group. Their PTG levels were assessed at 2nd day,15th day,and 30th day after surgery. Results In the end,a total of 142 cases were included. 78 cases were included in the smaller PTS group and 64 cases in the larger PTS group. The differences in PTGI-C scores of all patients at 2 d,15d and 30 d after surgery were no statistically significant. The PTGI-C scores of the patients in the smaller group of PTS were 34.95±25.60,31.96±22.20,and 37.77±25.32 at 2 d,15 d,and 30 d after the operation(F=2.704,P=0.069). The scores of the patients in the larger PTS group at each time point after the operation were 53.48±17.70,48.78±21.47 and 50.84±22.80 points(F=27.029,P<0.001). The difference in PTG levels between the 2 groups at each time point was statistically significant. Multiple linear regression results suggest that postoperative 2 d PTS has a statistically significant effect on postoperative 30 d PTG(β=0.272,P<0.05). Conclusion The lowest point of PTG within 30 days after surgery is 15 days after surgery. Therefore,at this time,medical staff should promptly identify people at risk of reduced PTG and carry out targeted interventions. When the PTS caused by the surgical event gets stronger,the PTG level will be higher. There is a positive correlation between them.

Objective To explore the effect of group pregnancy health care model on the delivery outcome of pregnant women with gestational diabetes. Methods The convenience sampling method was adopted to select pregnant women who had established a card and had regular obstetric check-ups in a tertiary A-level maternity and child hospital in Taiyuan from May to December 2018 as the research subjects. The random number table method was used to divide them into an experimental group and a control group,with 92 cases in each group. The experimental group adopted the group pregnancy health care model,and the control group received the conventional pregnancy health care model to compare the differences in delivery outcomes and psychological status between 2 groups after intervention. Results After the intervention,the fasting and 2 h post-meal blood glucose levels of 2 groups were lower than those before the intervention(t=-4.286、P=0.021),and the blood glucose level of the experimental group decreased significantly(t=-3.512、P=0.039);the natural delivery rate of the experimental group(χ 2=7.276),the perineum injury rate(χ 2=7.698) and the incidence of macrosomia(χ 2=5.821) were lower than those of the control group,and the difference was statistically significant(all P<0.05). The anxiety scale score in the experimental group was(37.63±5.62) points,and(50.67±8.69) in the control group. The difference between 2 groups was statistically significant(t=5.374,P=0.039);the depression scale score in the experimental group was(41.16±3.12)points and (54.13±4.22)in the control group,and the difference between 2 groups was statistically significant(t=7.125,P=0.044). Conclusion Compared with the traditional model,the group pregnancy health care model can effectively increase the natural delivery rate of pregnant women with gestational diabetes,improve the delivery outcomes,and reduce the occurrence of negative emotions.

Summary of best evidence of medication administration in patients with swallowing disorders

Objective To retrieve,appraise and summarize the best evidence of medication management in patients with swallowing disorders. Methods Using the method of evidence-based nursing for swallowing disorders patients,the problem of standard management of taking medicine was put forward. Evidence-based nursing methods were used to search UpToDate,BMJ,JBI,Cochrane Library,Ontario Registered Nurses Association of Canada,British National Institute of Clinical Medicine Guide Library,Scottish Intercollegiate Guidelines Network,National Guideline Clearinghouse,Guidelines International Network,Canadian Medical Association,American Heart Association/American Stroke Association,PubMed,CINAHL,UK NEWT website,Chinese Biology Medical literature database,Yimaitong,China Knowledge Network and other databases including all relevant documents from the date of database establishment to January 2021,covering clinical recommended practice,guideline,evidence summary,systems evaluation,expert consensus. The retrieved literature was evaluated by 3 researchers,and the literature that met the quality standards was extracted for evidence. Results A total of 12 pieces of literature were included,including 2 clinical decisions,2 guidelines,6 evidence summaries,and 2 expert consensused. The best evidence was summarized in 8 areas,including education and training of health care personnel,pre-medication screening and evaluation,multidisciplinary collaboration,compensatory methods,selection of oral solid dosage forms,administration of crushing drugs,gastrointestinal nutrition tube administration,and monitoring and recording of medication effects. Conclusion This study summarized the evidence of the normative management of medication for patients with swallowing disorders,so as to provide evidence-based bases for medical institutions to establish the practice standard of medication procedures for patients with swallowing disorders,and to ensure the safety of medication for patients.

The development and validation of a risk prediction model for peristomal moisture-associated skin damage

Objective To develop a risk prediction model of peristomal moisture-associated skin damage(PMASD),and to apply to clinical practice to validate its prediction performance. Methods The prospective study design and the convenience sampling method were adopted. A total of 329 patients from 4 hospitals in Shanghai were enrolled from November 2019 to September 2020,and predictors between the complication group(141 cases)and the non-complication group(188 cases) were compared using Logistic regression based on Lasso selection for model development. The goodness of fit of the model was verified by Hosmer-Lemeshow test. The discrimination was evaluated by the area under the ROC curve(AUC). The Bootstrap resampling was used for internal validation. From October to November 2020,109 patients were recruited for external validation. Results The incidence of complication in the development cohort was 42.9%(141/329),and 37.6%(41/109) in the validation cohort. The predictors that ultimately entered the prediction model were gender(OR=0.497),type of stoma(OR=4.338),height of stoma(OR=0.304),surgical wound in plate area(OR=3.479),plate attachment(OR=2.399),soft diet(OR=3.155),and semi-fluid diet(OR=3.325). The H-L test P=0.274;the AUC was 0.812. The Youden index was 0.500,with the sensiti-vity of 0.688 and the specificity of 0.798. While the external validation H-L test P=0.515,the AUC was 0.860 with the sensitivity of 0.610 and the specificity of 0.882. Conclusion The prediction model developed in this study has a good performance,which can provide a reference for clinical screening of high-risk patients with PMASD.

Objective To construct a nursing management model of a makeshift(Fang Cang) hospital under the epidemic of coronavirus disease 2019(COVID-19),so as to provide the experience and references for the establishment and the nursing management of the makeshift(Fang Cang)hospital. Method With the reference to the Jennings Disaster Nursing Management Model,we combined with the specific situation,formulated corresponding measures,connected all links,and finally constructed the nursing management model of the makeshift(Fang Cang) hospital under the epidemic of COVID-19 based on the in-depth analysis of the characteristics of nursing tasks in the makeshift hospital. This nursing management model includes 4 periods,namely the initial stage,the normal operation period,the closed period and the evaluation period. Results Under this model,the Jianghan makeshift(Fang Cang)hospital has been in effective operation for 34 days with a total of 1 564 beds. A total of 1848 patients were admitted and treated,with 1 327(71.81%) being cured and discharged and 521(28.19%) being transferred,and with no death. 675 nurses participated in the rescue aid with no infection. A total of 5,020 nucleic acid tests and 2,409 CT examinations have been completed,which made Jianghan makeshift(Fang Cang) hospital the largest hospital with open beds,cumulative admissions and discharge,nucleic acid monitoring and CT examination. Conclusion The establishment of the makeshift(Fang Cang)hospital under the epidemic of COVID-19 can effectively deal with public health emergencies. A highly effective nursing management model can help with the smooth operation of the makeshift(Fang Cang)hospital,which plays an important role in the combat against the epidemic.

Objective To summarize the practice experience of emergency nursing management of general hospitals in response to the outbreak of infectious diseases,so as to provide references for nursing managers to deal with major emergencies scientifically. Methods During the outbreak of coronavirus disease 2019(COVID-19),the nursing service department of our hospital,under the unified deployment of the epidemic prevention and control command,quickly set up the emergency management team of the epidemic prevention nursing organization and launched the emergency management plan. With cooperation of the implementation of emergency management,with continuous management of nursing quality and patient safety,multiple measures were taken to ensure the effective prevention and control of the epidemic. Results Under the unified guidance of the emergency management team of the nursing department,more than 2 800 front-line support nurses in our hospital successfully assisted and completed the critical rescue,inspection,diagnosis,treatment and transferring of fever outpatients in fever emergency department,isolation wards,Fangcang shelter hospital and other front-line clinic departments. Conclusion It provided references for guiding nursing staff at all levels to carry out scientific prevention and control of epidemic and guaranteed refined management by formulating emergency management plans of nursing organization and constructing precise management strategies.

Objective To describe the development and evolution of venous thromboembolism(VTE) nursing research in China,and to reveal research hotspots and frontiers. Methods Journal articles from CNKI and WanFang Database from the inception to 2019 were searched and retrieved with keywords of venous thromboembolism and nursing. CiteSpace software was used for the bibliometric and cluster analysis. Results The volume of papers issued in the nursing research of VTE is on the whole increasing trend. The direction of VTE nursing research involves the identification of high-risk groups and the development of nursing preventive measures. Among them,the elderly,the patients after joint replacement,the patients with malignant tumor and the patients after PICC catheterization have always been the research focus of the prevention of VTE. Conclusion As one of the specialized nursing direction,the nursing care of patients with VTE has been given more attention. Future research can explore effective methods in order to improve nurses’ knowledge for prevention of VTE,and develop intervention programs to promote the “flawless” VTE prevention

With the accelerating of aging in the world,living in institutional care facilities is one of the main options for the elderly. However,adaptability problems were commonly reported in the elderly as their relocating to institutional care facilities with the environment changes. Previous studies have shown that the elderly with adaptability problems may result in physical and psychosocial changes,which significantly influenced their quality of life. In previous studies,several evaluation tools were developed to measure the adaptability of the elderly,and the influential factors were explored,such as personal willingness,care quality of long-term care institutions,family support,and social support. This article focuses on the adaptability of the elderly in institutional care facilities,reviews the relevant research on it,and analyzes its concepts,current situation,evaluation tools and influencing factors. It also proposes the corresponding intervention strategies through the following 4 systems:social support system,institution system,family system,and personal system,providing references for improving the adaptability of the elderly in institutional care facilities and improving the quality of life of the elderly in institutions.

Objective To develop the communication training scheme for emergency nurses to respond to patients with attempted suicide,and to explore the effects of application. Method Under the guidance of American Suicide Prevention Resource Center’s guideline and incentive interview techniques,40 emergency nurses were selected to receive three sessions of communication training. The effectiveness of the training was evaluated by the verbal and social interaction questionnaire and nurses communication training feedback questionnaire. Results After training,communication frequency and importance scores of emergency nurses increased;difficulty scores decreased.The differences were statistically significant(t1=9.591,t2=5.124,t3=-4.494,P<0.05). The approval degree to nursing from patients and doctors approval was higher than before. Conclusion Communication training can enhance communication ability of emergency nurses to respond to patients with suicide attempt,improve recognition of patients and doctors towards nursing,reduce suicidal ideation of patients and promote the hope of life.

Investigation on occupational exposure and protective behaviors of nurses at emergency departments in tertiary hospitals in Beijing

Objective To understand the occurrence and protection status of three occupational exposures:needle-stick injury by nurses,blood exposure and workplace violence in the emergency department in tertiary hospitals in Beijing. Methods According to the schedule of the emergency department of each hospital,one group of nurses were selected from each hospital in 21 tertiary hospitals in Beijing to participate in the online survey. Self-designed online questionnaires were distributed and completed anonymously,including general information,the occurrence status and protective behaviors of needle-stick injury by nurses,blood exposure and workplace violence. Results A total of 1,150 questionnaires were collected,of which 1,102 were valid,with an effective rate of 95.83%. In the past year,the incidence rates of at least one needle-stick injury by emergency department nurses,blood exposure during blood collection,verbal violence,physical violence and sexual harassment were 37.8%,36.4%,68%,12%,and 5.3%,respectively. In different working areas at emergency department,the occurrence of needle-stick injury(χ 2=16.805,P=0.01),verbal violence(χ 2=98.509,P<0.001) and physical violence(χ 2=79.916,P<0.001) were different. Among nurses with occupational exposure,92.8% were able to squeeze blood after needle-stick injury,flush with flowing water and disinfect;69.8% were able to flush and disinfect exposed parts after blood exposure in the process of blood drawing. Among nurses encountering workplace violence,41.3% explained patiently,and 30.0% adopted tolerance and avoidance. In addition,the reporting rate of all three types of occupational exposure was low(less than 30%). Conclusion The incidence of occupational exposure in emergency department is relatively high. Nursing managers should attach importance to the construction of the work environment in emergency department,strengthen the training of pre-exposure protection in blood collection process and workplace violence,and strengthen the reporting and management of the three types of occupational exposure.

Objective To investigate the current status of male nurses’ disaster preparedness and analyze its contributing factors.Methods A total of 270 male nurses from 53 hospitals in Hubei Province were investigated with the Chinese version of Disaster Preparedness Evaluation Tool.Results The total score of disaster preparedness of male nurses was(162.87±40.53). Among three dimensions of the tool,personal preparedness dimension had the lowest score. Disaster rescue training,experience of disaster rescue,and clinical department were influencing factors of disaster preparedness(P<0.05 for all).Conclusion Disaster preparedness of male nurses is in the middle level and disaster training should be provided.

A survey of non-psychiatric inpatients suicides from 45 general hospitals in Hubei Province,China

Objective To understand the characteristics and influencing factors of suicidal behaviors of non-psychiatric inpatients in general hospitals in Hubei Province.Methods Using stratified-typical-target sampling method,50 general hospitals in Hubei Province were selected according to the ratio of 1∶1. All adverse event reporting system databases received between January 1st,2015,and December 31st,2017 were collected and analyzed to identify any events that involved suicide or suicide attempts occurred in the above hospitals.Results Totally 169 cases of non-psychiatric inpatients suicides were reported in 45 general hospitals. The incidence of inpatients suicidal behaviors in general hospitals was 3.48/100,000 admissions,the incidence of inpatients suicidal behaviors in secondary hospitals was 1.10 times higher than that in tertiary hospitals. Inpatients whose age≥60,with primary or junior high school education,unemployed or peasants,suffering from malignant tumors or chronic diseases,were more likely to commit suicide. Suicidal behaviors in hospitalized patients were more common in the spring,at 00:00-07:59 and within 1 week,mainly in the ward. The main suicide way was jumping from the buildings. The suicide completion rate was as high as 49.1%. Eleven hospitals had serious medical disputes due to suicide. The causes of suicide were mainly physical illness,family relationship,and family economy.Conclusion The incidence of inpatients suicidal behaviors in general hospitals is high. Administrators should provide training for healthcare providers on characteristics and influencing factors of inpatients suicidal behaviors,to improve focus and attention of healthcare providers towards suicide among inpatients.

Objective To establish a quantified performance appraisal system for nursing in children’s hospital operating room and analyze its clinical effects of application.Methods Through quantitative appraisal calculation method,a quantified performance appraisal system based on conventional nursing skills,together with specialized surgical skills,was established and applied to clinical practice.Results Surgical nursing quality was improved(P<0.001); nurses’ satisfaction with the performance appraisal system increased(P<0.05); the average number of daily operation for each nurse was increased,and the number of rest application for each week was decreased.Conclusion The quantified performance appraisal system for nursing establishes an objective and unified standard for operating room nurses,improves the quality of surgical nursing,mobilizes nurses’ work enthusiasm,increases nurses’ satisfaction and working efficiency,which has a positive impact on nursing management in operating room.

The correlation between complications of bed rest and quality of life after discharge among hospitalized stroke patients

Objective To describe the correlation between common complications of bed rest(pressure ulcer,deep vein thrombosis,pulmonary infection,and urinary tract infection) during hospitalization and the health-related quality of life after discharge among bedridden patients with stroke,in order to provide reference to form specific strategies. Methods A total of 3 238 stroke patients from 25 hospitals in 6 provinces or cities were recruited by convenience sampling. Common complications of bed rest during hospitalization were measured by case report form,and quality of life after discharge was measured using the EQ-5D scale by telephone interview. Results Among the 3 238 bedridden stroke patients,626(19.33%) patients experienced at least one of the major complications,including pressure ulcer(80,2.47%),deep vein thrombosis(29,0.9%),pulmonary infection(500,15.44%),and urinary tract infection(91,2.81%). Patients with any of the four complications during hospitalization reported more problems in all EQ-5D dimensions(Mobility,Self-Care,Usual Activities,Pain/Discomfort,and Anxiety/Depression) and had lower mean EQ-VAS scores than those without any complications. The occurrence of any complication was significantly associated with the proportion of reported problems in all dimensions after adjustment for confounding variables. Conclusion Common complications of bed rest were significantly associated with quality of life among stroke patients. Prevention of complications is vital to improve quality of life and reduce burden of disease for stroke patients.
